I have not read this myself, but this book by Kathleen Ryan and Daniel Oestreich was brought to my attention.

"Driving Fear Out of the Workplace" (1991, updated 1998, ISBN 978-0787939687) describes their study of in-depth interviews with managers and employees in diverse companies to explain how to follow through on Deming's advice. Among other things they discuss that in a high trust environment (culture) there are significant benefits to individuals, work groups and organisations in being able to discuss the undiscussables.
