Do all accidents have causes? This book sheds an interesting and quite different way on the way we often discuss accidents. Moving away from a Newtonian search for broken parts, this fascinating book explores complexity and how accidents (or just events) happen in complex systems. In these systems failure does not just happen through a simple linear cause and effect relationship where the effect is proportional to the cause, in complex systems failure can happen in unexpected, random ways with consequences that are exponential to the initial event. An important view on failure because our world and the systems we work with are getting more and more complex.
